Welcome to the Caldwell College Student-Athlete Information Page.
Student-Athletes can check this page for upcoming important dates such as
physicals or mandatory meetings. Athletic department policies also can be
accessed from this page.
Student-Athlete Physicals
All Caldwell College student-athletes must have a physical examination
before participating in any athletic activities associated with the
College's intercollegiate athletics programs. In order to receive a
physical, student-athletes must submit completed medical forms before the
physical. There are three forms: Personal and Insurance Information Form,
Pre-participation Physical Form, and
Liability, Risks,
Concussion, Bollinger Signature Form.
All student-athletes must complete and return all three
forms. Click on the links below to download the forms for
printing. Complete the forms and bring them in the day of your physical,
along with front and back copies of your insurance card. You
will not be able to fill out these forms once you arrive on campus for the
physical. NO FORMS, NO PHYSICAL, NO PRACTICE!
